Any material misrepresentation on my part shall be a sufficient ground for the denial of this scholarship/grant-in-aid application.

I consent and authorize the Provincial Scholarship and Grant-In-Aid Committee (PSGIAC) and the Kabugwason-Paglaum Unit of the Provincial Government of South Cotabato to process my personal and sensitive information contained in the application form and in the supporting documents I submitted for the purpose of verifying my identity, and to assess and evaluate my eligibility for the scholarship and grant-in-aid programs. I acknowledge that my personal data collected may be shared to authorized individuals to aid them in the selection process for the scholarship and grant-in-aid programs.

I understand that the Provincial Scholarship and Grant-In-Aid Committee (PSGIAC) and the Kabugwason-Paglaum Unit of the Provincial Government of South Cotabato will strive to keep my personal information confidential and will retain my information solely for the fulfillment of the above-mentioned purposes.

If my application for scholarship/grant is successful, the Provincial Scholarship and Grant-In-Aid Committee (PSGIAC) and the Kabugwason-Paglaum Unit of the Provincial Government of South Cotabato will retain my personal data even after my inclusion in the program is terminated.

If my application is denied, the Kabugwason-Paglaum Unit of the Provincial Government of South Cotabato will retain my information. Further, I am allowing the Kabugwason-Paglaum Unit of the Provincial Government of South Cotabato to contact me using the information provided in the application to acquire my consent for the purpose of re-processing my application in other grants/educational assistance programs offered by the Provincial Government.
